Certified true to the original
Certified true to the original is the note by which someone confirms, under signature, that a document matches the original exactly. On a certified translation, the translator declares in this way that the translated text faithfully reproduces the original.
When it appears
The note appears in two situations. On a certified translation, the translator confirms conformity with the source document. On a notarized copy, the notary confirms that the photocopy matches the original presented.
Why it matters
Institutions do not accept translations or copies without this confirmation, because it establishes the responsibility of the person who made the translation or the copy. Without the note of conformity, the document remains a plain photocopy, with no official value.
